Getting Referred to the Clinic

Private Referrals

All our private patients are seen in the Lisa Sainsbury private patient wing which is on the Hammersmith Hospital site.

We are happy to accept referrals from your gynaecologist or GP and these can be posted or faxed to any of the consultants to the address below. The referral letter should include any relevant tests or investigations you and your partner have already had done such as hormone and semen analysis, HSG or laparoscopy.

As a private patient you will not be subject to the eligibility criteria that NHS patients have to meet, but doctors do have to assess on an individual basis whether or not it is appropriate to treat you. You may wish to discuss this with your doctor at your first consultation.

Private and overseas referrals should be clearly marked “PRIVATE” and addressed to:

Overseas Patients

Every year we see a number of private patients from overseas. Ideally you will still need to be referred in to us by a doctor but we can also accept a personal referral. Please post or fax your referral letter to the above address. It is important that you bring your medical records with you, and do not post them to us.
If you require a laparoscopy or hysteroscopy we will make every effort to carry out this procedure within a few days of your consultation to cut down on your accommodation costs in the UK. Although there should be no real discomfort afterwards, you must not travel outside the UK for at least 48 hours after the procedure. If you require IVF, you may need six weeks minimum in the UK and ideally you should be in London at least three days before the start of your period. However, we may be able to arrange for you to arrive much nearer to your start date if you contact us in advance. Male partners will need to be here to give a sperm sample before treatment. Whilst undergoing treatment you may live at any place convenient to you within reach of
the hospital.

We are happy to advise you on accommodation and transport to the hospital and make arrangements for you in advance of your consultation if necessary. We regret that we are unable to supply letters for visas, so please ensure your documents are in order before making an appointment.


NHS Referrals

Our NHS patients are seen in the new Queen Charlotte’s & Chelsea Hospital gynaecology outpatient department, which is on the Hammersmith Hospital Du Cane Road site.

Most patients are referred to us by their family doctor (GP) or consultant gynaecologist. Your GP will need to see you first and may ask for a few tests to be done before making a formal referral to the fertility clinic. Once this is received we will process the referral and send you an appointment as soon as we can, usually for the next available appointment. In some cases your local Primary Care Trust may ask that you are seen at another (local) hospital first. If in doubt, call on 020 8383 4908 (see NHS Funding section).

Unfortunately, we cannot categorise between routine or urgent appointments and we send out the appointments on a first-come first-served basis. However, our waiting times are currently well within the 13 week NHS standard.

NHS Funding

NHS Patients contemplating IVF treatment for the first time are often confused by the rules surrounding the criteria needed for their successful funding. While private patients can start IVF treatment as soon as they are given the go-ahead by the consultant and IVF doctor, NHS patients must meet various criteria before their name can be added to the waiting list.

Many GPs are unable to refer patients for IVF as their Primary Care Trust (formerly Health Authority) do not fund any assisted conception. Some PCTs do fund IVF, but have a preferred hospital that they like to use which limits patients to using that hospital as a funded NHS patient or going to a hospital of their choice and having to pay privately.

Where Primary Care Trusts do fund IVF at the Hammersmith, they often ask that the initial investigations be undertaken at a local hospital first before patients are sent to us as tertiary referrals (these are referrals made by one hospital consultant to another). Once they have been referred we then have to ensure that they meet the relevant eligibility criteria of age, children from previous relationships, referral source and number of previous cycles. Nearly all PCTs have different eligibility criteria, so it is worth checking with us as early as possible if you are entitled to treatment.

The responsibility for making sure patients receive the treatment available to them under the NHS falls to the service manager, who is available to discuss the terms of NHS IVF funding on 020 8383 4908 as well as funding for ICSI, preimplantation genetic diagnosis, egg donation and embryo freezing.


Your First Consultation

We ask that all patients try to attend the clinic at least ten minutes before their appointment time as there may be some administration and pre-appointment checks that need to be carried out first.

Your first consultation may take around 30 minutes, depending on whether you have had any investigations before. We ask that you attend with your partner and bring any other information we may need such as previous test results.

The consultant will go through a detailed gynaecological history and arrange appropriate investigations for you and your partner. You may be referred for other tests such as the HSG test (see Glossary) or for a hysteroscopy or laparoscopy. This will all be explained to you in the consultation.

If you think you are running late or are unable to attend the appointment at all, please let us know as soon as you can so that we can offer the time slot to another patient.






Commencing Treatment - Routine Monitoring

We open at 7.00am to allow our patients the option to attend the Clinic before their working day, helping to reduce work absence and stress. Regular Vaginal Ultrasound scanning and hormone assessment in the form of blood testing is used to monitor treatment cycles. This allows us to identify any potential problems arising during the treatment.


Medical Supervision

High medical staffing levels mean that IVF doctors are always available to answer queries and to see emergency patients. Weekend cover for Saturdays and Sundays is also provided.

One IVF doctor is allocated each morning to the operating theatre to perform egg recoveries. The Hammersmith unit is unusual in having a senior specialised anaesthetist also present for all operative procedures. Although this does not cost our patients any extra, it guarantees a safe service and ensures that patients have appropriate pain relief or anaesthesia at all times.


Nursing Team

We are fortunate in having a large nursing team specialising in the field of Reproductive Medicine. The nurses are very aware of the stresses and strains experienced by people undergoing reproductive treatments. They provide nursing cover seven days a week in the IVF unit to enable patients to have daily support, should they require it. There is also a nurses telephone helpline.

The nurses are involved in all stages of the treatment cycle and run the preliminary co-ordination clinic for new couples, providing the practical advice necessary. They also care for the patients during monitoring, taking blood and teaching self-injection techniques, and they will be with you on the day of egg recovery in the operating theatre and recovery room. The support that they provide to partners also helps to relieve anxieties about the treatment.


Counselling

We encourage everyone to make use of our free and confidential counselling service. It can be extremely difficult to cope with infertility and you may already have found counselling helpful whilst undergoing investigations. It becomes even more valuable before, during and after assisted conception treatment. You may be experiencing a great deal of stress or emotional difficulties or you may simply want to take special care in preparing for your treatment. Extra support during your IVF cycle can be important and counselling may really help if you receive bad news at any point.
