We try to perform all our fertility surgery whether removal of fibroids, ovariancystectomy or adhesion division, using microsurgical techniques. Microsurgery allows thebest healing with the least scar tissue.
All four surgeons at the Hammersmith Hospital are exclusively reproductive surgeons dealing with all types of microsurgery, laparoscopic and hysteroscopic surgery. The large throughput of complex cases allows this extensive experience to be maintained. As a result of this we train nearly all the sub-specialist trainees in Reproductive Medicine throughout Southern England.
Tubal Surgery
Tubal surgery can be an extremely successful in restoring fertility. We use microsurgical techniques, including an operating microscope, to restore the fallopian tubes to a more normal condition, and hence allowing natural, normal conception.
Tubal microsurgery can have significantly higher conception rates than IVF and is the first option to be discussed with patients with certain forms of tubal damage. More microsurgery is performed on an annual basis at the Hammersmith Hospital than anywhere else in Europe and probably the world.
One benefit of tubal microsurgery is that it allows the patients to fall pregnant naturally without medication or intensive monitoring. Women generally spend only two nights in hospital after microsurgery and recovery isoften quicker than with other laparotomy procedures.
Reversal of Sterilisation
This most successful form of tubal microsurgery gives intrauterine pregnancy rates of up to 86%. If the patient has been sterilised with older techniques such as Falope rings or by diathermy, providing there is a healthy portion of fallopian tube, reversal can still generally be performed. The success rate is reduced when a greater amount of tube is damaged and after diathermy sterilisation is often about 60-65%. Microsurgery is only impossible when the tube is damaged in its entirety.
The Andrology Team
The hospital is unusual in having such a large specialist andrology laboratory dedicated to testing sperm. Hammersmiths service is probably unique and we can approach quality assessment and appropriate advanced testing of semen samples which are often difficult or impossible for the non-specialist pathology laboratory. The andrology service is adjacent to the IVF unit and a walk-in service is available each weekday (from 9.00am- 12.00am). Comfortable waiting and sample bedrooms are provided next to the laboratory and female partners are welcome to attend.
